Such uses may include but are not limited to, advertising and promotional materials, brochures, newsletters and electronic media including the ZERO GRAVITY DANCE CO. website. Yes - I give unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by ZERO GRAVITY DANCE CO. for a variety of purposes and that these images may be used without further notification. LIABILITY WAIVER I hereby waive all present and future claims for any loss, damage or personal injury sustained by the above-mentioned student (hereinafter referred to as “the student”) including but not limited to, physical injury sustained by the student during classes, performances or competitions, contact with a person with COVID-19 or other infectious sickness/diseases and I further release, remise and completely discharge the promoters, directors, principals and employees of ZERO GRAVITY DANCE CO. and any competition or performance attended by ZERO GRAVITY DANCE CO. from any liability whatsoever for any injury or loss caused as a direct result of any act or omission by any of them. I hereby acknowledge and agree to all of the terms and conditions set out in this registration package as well as those in the Student Code of Conduct. AGREEMENT FOR PARTICIPATION I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across-the-floor combinations, tumble track, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. All dancers are provided with a cubby for their belongings to be stored while they are in class. Zero Gravity Dance Company is not responsible for the loss of personal property. I have received the Pre-Competitive student registration package, and agree to adhere to all the content stated therein including: • Pre-Competitive Team Contract • Tuition, Fees & Payment Information • Code of Conduct • Dress Code • Calendar • Media Recording/Liability Waiver I agree to be responsible for reading studio correspondence and respecting deadlines. I hereby acknowledge that I have read the statements above and agree to participate accordingly. I understand that participating in any classes or competitions may expose me or my child/ren to COVID-19 or any other sickness/disease, and I accept all risks. I understand that once all offers and technique choices are accepted, I am committing to pay the amounts owing on time according to my payment plan.
